The Home-Based Home Visitor works directly with families of infants and toddlers to support school readiness through weekly home visits and group socializations. This position partners with parents as their child’s first teacher and connects families to community resources.

Essential Duties

  • Conduct weekly 90‑minute home visits with assigned families
  • Plan and facilitate two group socializations per month
  • Develop individualized lesson plans and Family Partnership Agreements
  • Support parents with child development, health, nutrition, safety, and mental wellness
  • Promote positive parent‑child interactions using research‑based practices
  • Assist families in accessing community resources and follow up on services
  • Maintain accurate documentation and data entry
  • Collaborate with team members and participate in required trainings and meetings
  • Community travel required

Qualifications

  • Required: Associate degree/equivalent coursework in Early Childhood Education, Social Work, Human Services, Counseling, or related field
  • Preferred: Bachelor’s degree in a related field
  • Experience: At least 1 year working with infants, toddlers, and families
  • Valid driver’s license required
  • Infant/Child CPR certification (or ability to obtain)
  • Strong communication, organization, and basic computer skills
